IMPORTANT INFO REGARDING PASSPORTS: The U.S. Government will soon require all cruise passengers (including U.S. citizens) to be in possession of a valid passport when traveling on any Carnival cruise. Foreign nationals should contact their respective governments to obtain further details.

PASSENGER INFORMATION: Important -- We must collect FULL LEGAL NAMES (no nicknames) and accurate DATE OF BIRTH as required by Homeland Security. If you are traveling with a Bill or a Chris or a Jen (for example) please verify that their legal name is not William or Christopher or Jennifer.
If making reservations for three or four passengers, please submit an additional reservation form, and indicate the name of the Lead Passenger with whom the third and fourth passengers are affiliated, within the Additional Passenger Info field. A valid email address is required for the Lead Passenger. Whenever possible, please include a unique email address for each additional guest.

CABIN SELECTION: If you would like to reserve more than one cabin, please complete this booking, then repeat the process for each additional cabin reservation.
NOTE: Carnival Cruise Lines will spend $250 million to upgrade and refurbish its Fantasy-class ships, beginning with The Imagination this fall. Children's water parks; a new design and features in the pool areas; and a new adults-only deck will be part of what Carnival is calling an "extreme makeover" of the eight ships, that will include renovations to almost every guest area, including all passenger cabins!

PRICING: Prices are in US Dollars (USD) per-person based on double occupancy, and include all cruise fees and mandatory port fees. Additional guests above double occupancy start at $349/pp. Call or email for complete details.
Prices do not include taxes, gratuities, ticketing, airfare, transfer fees, travel insurance, or other fees. An additional $137/pp for mandatory government taxes, prepaid gratuities, ticketing and credit card processing fees, will be collected at the time of processing.
Vince Neil's Motley Cruise/MK Cruises reserves the right to increase published prices without notice.
PAYMENT:
- 50% of cruise fees due at time of reservation and;
- Additional 25% of cruise fees due on August 10, 2007 and;
- Final balance of charges due on October 5, 2007
- Past-due payments are subject to a minimum $35 late fee
AGE REQUIREMENTS: Carnival shall refuse boarding to any Guest under the age of twenty-one unless: (1) the Guest is traveling in the same stateroom with an individual twenty-five years or older; (2) traveling in the same stateroom with their spouse; or (3) traveling with a parent or guardian in an accompanying stateroom. Proof of age and/or proof of marriage are required. Carnival shall not be liable to make any refunds or for any damages with respect to any Guest's failure to provide proper proof of age or marriage or otherwise comply with this provision. Infants are REQUIRED to be at least 4 months at the time sailing. Pregnancy: Women who will be entering their 27th week of pregnancy at time of sailing will be prohibited from taking a cruise. Ages calculated from the date of sailing.
CHANGES & CANCELATIONS: There is a strict cancellation policy. All changes, transfers or cancellations may be made only by the Lead Passenger. Any request to change the name of the Lead Passenger shall be deemed a transfer of reservation. Any transfer of reservation is subject to the written consent of MK Cruises Inc. and requires payment of a transfer fee of $199.00 per reservation in advance before such transfer shall be effected. Please read the terms of the Ticketing Contract for further details.
INSURANCE: Vince Neil's Motley Cruise wants you to have the best vacation possible, with nothing to think about except how much FUN you're going to have. Cruise insurance can be purchased to cover you in the event you must cancel. The Lead Passenger and each Guest is strongly urged to consider travel insurance to protect your trip. TERMS & CONDITIONS: By making the reservation for the Vince Neil's Motley Cruise or by the acceptance and/or use of the Ticket Contract by the persons named thereon as Guests, the Lead Passenger and each Guest on the same reservation, acknowledges he or she has read, understands and agrees to be bound by each and every term, condition and provision of the Carnival Cruise Lines LLC. passenger ticket contract and agrees to be bound thereby.
